Incident Logs

How to create an incident log, how to locate and manage previously created incident logs.

You can use the Incident Log section of Atlas to keep track of any main incidents that happen in your business, to either employees or other people coming into contact with the organization, such as customers or suppliers. Incidents include near misses as well as accidents.

IMPORTANT: The below information is intended for Users with full access to this service. If your access to this area only allows you to add incidents, you should speak to your Manager and request the additional permissions.


Accessing your Incident Logs

From the left-hand navigation menu, simply click Health and safety > Incident log to access the dashboard for this service.

From here you will have quick access to your organization's Pending, Approved and Archived logs. You will see at a quick glance how many days since your last incident, and you can filter the various categories by sites, types, year created and status.

You can manage your existing Incidents from this page. By clicking the Actions icon (...) next to each Incident, you will have the following options:

  • View

  • Update

  • Archive (You cannot archive an Approved Incident)

  • Reinstate (Only available for Archived incidents)


Creating an Incident Log

Start by clicking the Log Incident button in the top-right corner. There are six sections to complete, and any required fields will be marked with a red star. Changes are automatically saved when you navigate between sections.

1. Person reporting

This section is to record the person reporting the incident. It will autofill the details of the user who is currently logged in and their associated Site, but you will have the opportunity to update name and address details before continuing.

2. About the incident

First, you will have to select the Incident type from the dropdown menu. There are multiple options available, broken down into seven categories:

  • Behavioural

  • Dangerous Occurrence

  • Environmental

  • Fatality

  • Injury

  • Near Miss

  • Occupational Diseases

You will then be asked to record any information on the injuries sustained, including type and injured body parts, descriptions of the incident and injuries, and dates and locations. You will also be asked to provide information on witnesses and enforcement intervention, if any.

3. About the affected party

Use this section to record the details of the person affected by the incident. From the Affected party dropdown, select from:

  • Contractor

  • Employee

  • Member of Public (including children)

  • Resident

  • Self-Employed Person

  • Service User (including children)

  • Student

  • Young person

  • Other

IMPORTANT: The additional details you will provide will vary based on your selection.

4. Reporting

In this section, you will answer questions on the left-hand side. The content of these questions is based on the jurisdiction where incident occurred, as reported on Step 2. On the right there will also be other relevant information and recommendations to consider.

IMPORTANT: Depending on the answers you provide, you may be instructed to immediately notify the relevant governing body within a certain timeframe, and you can click the Report Now button to begin the process. If you have questions or concerns, contact your health & safety consultant directly or call the main Health & Safety Support Line.

5. Formal investigation

IMPORTANT: If an incident requires reporting to the relevant jurisdiction, or may lead to a complaint or a personal injury claim, then please ensure you also carry out a formal investigation using this page.

Toggling on the option for Is a formal investigation required? will populate all relevant fields for your formal investigation. While not all fields are requires, it is highly recommended that you be as detailed as possible.

6. Preview

In the final step, you will be able to preview the finalized incident document in its entirety - This will be your opportunity to check that the information is accurate, and you can navigate back to previous steps to make adjustments if necessary.

In reaching this step, your incident it will be in the Pending status. Clicking the Approve button will mark the incident as approved. You can also click Save Incident to Atlas if you want to keep a PDF version in your Documents area or Export Full Review button to download it as a .PDF document to your device.
